THE POSITION

The Case Manager is responsible for supporting children, youth and families who are referred to RISE Idaho Family Services to ensure they have the coordination and resources necessary to be successful. Services include, but are not limited to, permanency support, treatment foster care, and support for youth transitioning out of foster care/kinship/adoption into adulthood.

The Case Manager will coordinate and collaborate with the Program Supervisor in making regular contact with families initially and ongoing to assess the need for support/services. Case Manager will provide relevant, community-specific referrals and resources to families for a variety of supports, including but not limited to, mental health resources, developmental services (Intervention Services, Occupational Therapy, Physical Therapy, Speech Therapy), support groups, etc.

Case Manager will be well versed in adoption, foster care, and/or permanency related issues, trauma-informed care and cultural competency and will use this knowledge in delivering high-quality, professional services and supports to families statewide. Services are delivered in person, over the phone, through email and through video conferencing with a child/youth and members of the treatment team, including their family.

POSITION EXPERIENCE & EDUCATION REQUIREMENTS

  • Bachelor's Degree in Social Work, required
  • Two (2) years' of full-time, paid experience in adoption or foster care services
  • LSW, preferred
  • Certification as Targeted Care Coordinator in Idaho, preferred
  • Current First Aid/CPR Certification, preferred
  • Excellent verbal and written communication skills.
  • Ability to maintain confidentiality of sensitive company information including but not limited to pay, benefits, compensation and financial information.
  • Ability to greet the public by telephone and in person in a professional manner.
  • Interpersonal skills necessary for effective communication and interaction with referral sources, clients, prospective foster parents, foster families, outside agency personnel and other care providers when gathering and exchanging information. This level of interpersonal skills requires a great deal of tact and diplomacy. Being able to openly discuss sensitive subjects with prospective foster parents and current foster parents in order to effectively address, assess and intervene as necessary to maintain the viability of the home for placements.
  • Ability to write all necessary materials and communications relevant to the position and interpret policies, procedures, and other required information.
  • Excellent organizational and time management skills, demonstrate attention to detail, exhibit patience and perseverance in completing tasks, demonstrate ability to handle multiple tasks and timely follow-through skills.
  • Self directed, works with minimal supervision.
  • Knowledge of state, licensing and contract expectations.
  • Workable knowledge of Microsoft Word, Excel, & Google Platforms

ESSENTIAL JOB FUNCTIONS

CASE MANAGEMENT

  • Initiate telephone, email or face-to-face contact with each child/youth IDHW assigned based on requirements per contract or licensing
  • Initiate ongoing an frequent communication with the childs IDHW assigned FACS social worker and/or DBH worker, school personnel, mental health treatment providers, the childs parents or guardians, and other parties involved in the childs life and treatment
  • Participate with the child and family team in developing, monitoring and reviewing of treatment plans
  • Work closely in collaboration with team members and other community providers to ensure the child and family have access to the information and resources necessary

DOCUMENTATION

  • Chart all case management activities accurately and timely to ensure payment from insurance/payer sources

MISCELLANEOUS

  • Act as a point of contact and build influential relationships during the selection process
  • Promote RISE, its mission, vision and values.
  • Act as an appropriate role model in using appropriate language, dress, manners, and in showing respect.
  • Travel up to 25% of time
  • Participates in meetings and other activities internal to RISE; specifically, but not limited to monthly Management Team meetings. Works with training dept. to effectively create and train interviewing techniques. Collaborates with management and administrative staff as needed.
